What is FTM Top Surgery?

Female-To-Male or FTM top surgery is an elective surgery that removes your breast tissue to make the chest look more manly. It also reduces the size of the areola to improve the aesthetic results.

Depending on your anatomy, your Dr. Agha may choose from various surgery techniques to perform the procedure.

However, the decision to do so should be taken seriously. Not only will the procedure leave noticeable and permanent scars on the chest, but it’s also irreversible.

How is the Procedure Done?

Based on Dr. Agha’s assessment of what will work best for you, he may choose from the following:

Periareolar Surgery

Periareolar surgery is an ideal option for patients with smaller chests or B-cup breasts with good skin elasticity.

It involves two circular incisions. The first is around the areola through which the areola is made smaller. The second concentric incision is made to remove excess skin. A purse-string closure technique will then be used to connect the skin to the areola to resize it. Through these incisions, the excess breast tissue is accessed and removed.

With this procedure, there’s minimal scarring, and nipple sensation is retained. However, these benefits may come at the risk of patient satisfaction because 40% to 60% of patients who chose this surgery need revisions later.

That is why it’s essential to choose an experienced and board-certified plastic surgeon like Dr. Agha to ensure the best results.

Keyhole Top Surgery

Keyhole top surgery is an option for patients with smaller breasts who have minimal hanging breast skin. Like periareolar surgery, this approach retains nipple sensation.

The keyhole Top Surgery involves an incision around the areola and then down vertically under it. Through these incisions, the areola is made smaller and the breast tissue is removed. This is similar to the Vertical Breast Lift technique, also known as the lollipop breast lift.

Double Incision Top Surgery

The double incision top surgery is the most widely-used surgical technique. It is ideal for persons with medium to large breasts.

First an incision is made around the nipple areola to make it smaller and to remove the nipple areola off of the breast gland, like a skin graft. Next, an incision is made at the mammary crease. This allows Dr. Agha to pull back the chest skin and expose the breast tissue.. He will then remove the breast tissue completely over the pectoral muscle. Next, the chest skin is redraped over the chest muscle and the excess skin is cut at the level of the mammary crease. The grafted nipple areola is then sutured at its new location.

One of the advantages of this surgery is having only two incisions, around the areola and at the mammary crease, instead of three. However, the nipples are removed before being resized and regrafted, reducing the sensation in the nipple-areola complex.

Finally, Dr. Agha may perform liposuction and liposculpture to create a more defined and masculine contour.

Buttonhole Technique Top Surgery

This is Dr. Agha’s preferred technique for medium to large breasts. Like the double incision top surgery, incisions are made at the mammary crease and over the chest to remove the excess breast skin, breast tissue and fat.

However, instead of nipple grafts, Dr. Agha keeps the nipple-areola complex connected to its blood supply and nerves through a thin stalk of breast tissue called a pedicle.

He then creates an opening on the chest skin where the nipple-areola complex attached to its pedicle will be brought through from under the skin, like how you’d insert a button through the buttonhole.

The nipple areola is then sutured to the surrounding skin and the mammary crease incisions are closed.

Dr. Agha highly prefers and recommends the buttonhole technique because it preserves nipple sensation and results in better nipple-areola size, pigmentation, and positioning.

This means that the patient enjoys a better aesthetic outcome while retaining the original sensitivity of the nipple-areola complex.

Again, Dr. Agha may perform liposuction and liposculpture to create a more defined and masculine contour.

Inverted-T Top Surgery

Ideal for patients with very large breasts who need a greater degree of skin removal, the inverted-T top surgery is similar to the Anchor-Type Breast Lift.

Through an inverted-T or anchor-type incision, the excess chest and breast skin are removed,, the breast tissue and fat are excised, and the nipple areola are repositioned on a breast pedicle, similar to the buttonhole technique.
