Basically speaking, large volume liposuction is the removal of a substantial volume of excess fat in one sitting. It can be done on various parts of the body, including the abdomen, hips, buttocks, thighs, arms, and neck.

But unlike regular liposuction, large volume liposuction can be recommended to overweight patients who are still in the process of losing weight. After all, the procedure can speed up the weight loss process, quickly helping patients feel more confident in their skin.

In the United States, the American Society of Plastic Surgeons (ASPS) has set the limit for large volume liposuction done in an ambulatory surgery center to only five liters of fat removal in one sitting. Anything more than this should be done in a hospital due to the increased risk of fluid shift.

According to studies, large volume liposuction has long-term health benefits, such as improving cardiovascular risk factors, metabolic balance, and insulin resistance.

Can You Remove More Than Five Liters of Fat?

Theoretically, yes — more than five liters of fat can be removed at one time. However, you have to be monitored in a hospital for possible complications.

To understand why such a thing isn’t advisable, it is crucial to know how large volume liposuction works.

For every cubic centimeter of fat harvested, an equal or higher volume of tumescent fluid is first injected into the patient’s fatty tissues. Afterward, the fat is liposuctioned.

Some of the tumescent fluid is removed together with the fat. Still, the rest is absorbed by the body, increasing the blood’s water level and altering the concentration of electrolytes in the bloodstream. Also, broken fat cells release certain electrolytes into the tissues.

All these create fluid and electrolyte changes in the bloodstream, which can have an adverse effect on the cardiovascular system. There is also the risk of increased blood loss the higher the volume of fat removed, which is why the ASPS has set a limit to large volume liposuction, at least in ambulatory surgery centers.

That said, not all physicians and plastic surgeons doing liposuction follow the recommendations of the ASPS.
